Medication Reminders not syncing between iPhone and iPad

I recently started using the Medication Reminders in the Health app. I’ve found that medications I’ve logged in one device are not reflected in the other device. This has been quite confusing and could end up being dangerous.

Both devices are logged in with the same Apple ID and iCloud account.

Is there a setting I’m missing somewhere for this?
