Is there any difference between a 1st & 2nd class medical?

I’m trying to understand this topic fully before my check ride. I know what each medical class qualifies you for but as for the actual contents of the exam and the passable parameters what is the difference (besides an EKG test if you’re 35+) ? If there is no difference, who would possibly apply for a 2nd class cert instead of a 1st?
