maintaining plants while replacing substrate?

i want to replace my substrate with soil and sand cap, right now it’s just sand mixed with gravel and my submerged plants are having trouble rooting since there’s no nutrients in the sand. my question is, how can i preserve the montecarlo plant i have carpeting on my current substrate?
