Crashing/ Graphics Errors and other Errors

I decided to go AMD maybe 5 years ago now and I've been having the same issues ever since. I can't tell what the issue is at this rate and I'm hoping someone has a fix. I will try my best to go over every part I've changed since then so maybe we can figure something out. I've reinstalled windows more times than I can count and I've tried every GPU driver since 2019. Current gear will be on the left, old gear will be on the Right.

GPU: Pulse Sapphire 5700xt Tomahawk Max, Nvida 960 2GB (this was my old gpu, still had dithering while gaming but I haven't caught a Green Screen with it since its recently stopped working on me, prob over 10 years Old I dont remember) I did try an AMD 580 but that one just didn't work at all.

CPU: 3900x 12 Core 3801 Mhz, Was using a 3700x and its the first thing I replaced (Was having like 20 fps on Low Graphics when Falled Order came out) I also tried a completely different 3700xt as well

Motherboard: X570S Aorus Elite RX, Ran two different B450 Tomahawk Max's because I thought maybe the Motherboard was bad

PSU: Gigabyte 750 Watt Gold 80 Fully Modular

Ram: 32 GB Ripjaws 3200 Mhz (They're not reading right so they're XMP'd to 3200 Mhz)

Driver: Name a driver no change

Chipset: Tried a few, currently on Latest

The issues are VERY game dependent ranging from USUALLY some extreme dithering followed by some really strange rendering (I'll show in the video since this is the current game running I'll try and get a video of all the issues ASAP but for now this should help) and if I'm playing Vermintide 2 or Final Fantasy 1-5 I'll just straight up greenscreen with an assortment of issues in Event Viewer. I've even had greenscreens while watching YouTube, no games open. Some games have really bad frame drops where I can go from like 200 fps to 20 fps for a few frames then its back to normal. I've seen this every single time I get a green screen which leads me to guess a CPU error however with 3 cpus I'm not so sure.A fatal hardware error has occurred.

I've also had the usual (the graphics driver has stopped responding, or w/e the actual wording is) so I really can't tell if its the GPU either.
